Use this form to indicate your Primary and Contingent Beneficiary(ies) and whether you wish to restrict payout. If you choose to restrict payout, then you must complete Section 4 for each beneficiary being restricted.

This form can be completed online. You can type your information directly into the form using your computer. When finished just print and sign. Or you can download and print the application. Once the form is printed, complete all the sections manually and sign. The choice is yours.
