Purpose of this form:
Complete this form to open a rollover IRA mutual fund account with John Hancock.

Instructions for using this form:

  • This form can be completed online. You can type your personal information directly into the application using your computer. When finished just print and sign.
    Or you can download and print the application. Once the form is printed, complete all the sections manually and sign. The choice is yours.
  • You will also need to complete a Termination and Withdrawal form. This form is available from your plan administrator. Complete Sections A and E (Participant Signature) of the form.
  • The minimum initial investment to open an IRA Rollover Account with John Hancock is $500.
  • It is always a good idea to save copies for your records. Forward the completed application as well as the Termination and Withdrawal form to:
